I'm really honored when people want one of my artworks on their skin forever. To make it easy on you I've come up with this simple release form. This form has everything you need to take to your tattoo artist to have one of my images tattooed. 

With this purchase you will get a written permission via email which you can show to your tattoo artist. All you have to do is download the form, print it out and fill in your name, and the title of the artwork you wish to have tattooed. The form is already signed by me.

You can choose from any available designs on my website.

Direct PayPal and credit card payments through the website aren't available now as I live in Russia. All payments are processed manually through my Boosty blog. How it works:

1. You fill your cart and create your order as usual, but without payment
2. I create a paid post in my Boosty blog at the amount of your order converted to Russian rubles (the only currency available on Bootsy) and send you a link
3. You pay for this post using your credit/debit card or PayPal account
4. I print/pack/send your order as usual
